WELCOME TO THE SCHOOL HEALTH CLINIC

Dear Potomac View ES parents and guardians,

My name is Tracy Parks and I am the new school nurse at Potomac View Elementary.  I am 2009 graduate of Marymount University school of Nursing and I have experience in Emergency and Urgent care nursing.  I am very excited for my new role here and to get to know your students!  

If your child has a chronic health issue please obtain the appropriate forms from the school office or clinic so that we can create a treatment plan that works best for your child! Also, the forms you may need are below so you may print them from home.

Fifth graders need Tdap for entry into Middle School
In 2006, the Virginia General Assembly passed a law requiring all sixth-grade students to have had a Tetanus, Diptheria, and Pertussis (Tdap) booster shot within the past five years in order to enroll in middle school. Without proof of this immunization, your child will not be able to enroll in school for next school year.
 
If the last shot was administered more than five years from the beginning of the upcoming school year, please have this immunization done before the first day of school. This shot may be listed as T, Td, Dtap, and/or Tdap. If your child is not 11 years old by the first day of school next year, your physician may choose not to administer the immunization until after the eleventh birthday. Please contact your physician regarding this situation.
 
Shots may be obtained from your doctor, military clinics, or the health department. 
 
Please send proof of the Tdap immunization to Potoamc View E.S. clinic as soon as it is done.
 
If you have any questions, please contact your School Nurse.
IS MY CHILD TOO SICK TO ATTEND SCHOOL?
Illness or Injury Exclusion Criteria R758-1

Reasons for which a child may be sent home from school or for a parent to keep the child home from school.

1. Fever of 100 degrees F and over-exclude until student has been fever-free for at least 24 hours.

2. Conjunctivitis (pink eye), strep infections, ringworm, and impetigo are all infections and must be treated with medications for 24 hours before returning to school.  Please do not allow affected students back before this time so that other students are not infected unnecessarily.

3. Rash of unknown origin (especially if accompanied by a fever).

4. Head injury.

5. Sever coughing or difficulty breathing.

6. Colds-a child with thick or constant nasal discharge should remain home.

7. Diarrhea or vomiting-exclude until child has been symptom-free for at least 24 hours.

8. Stiff neck associated with a fever and/or recent injury.

9. Inadequate immunizations with known disease outbreak in school.

10.  Refer to the VDOH "Communicable Disease Reference Chart for School Personnel" for other exclusions/information
MEDICATIONS
All medicine (over the counter or prescription) administered at school must have a medication authorization form on file in the clinic. Prescription medication must have a physician's signature on the form before it can be administered. All medications must be in their original container, with the child’s name printed on the label. Over-the-counter medications must be unopened. The form and medication must be brought to school by a parent or other adult, and will be kept in the school clinic.

STUDENTS ARE NOT PERMITTED TO CARRY MEDICATIONS IN SCHOOL UNLESS PREVIOUSLY AUTHORIZED

- Regular non-medicated Chapstick is allowed in school and ENCOURAGED to help keep your students lips moist during these cold winter months!


- There is not a "stock" supply of commonly used Over-the-Counter medications kept in the school clinic. All medication given to students must be provided by a parent along with the required form.
PLEASE REPORT YOUR CHILD'S ABSENCE
It is important that you call to report your child's absence as soon as possible, especially when it is due to illness. Please give basic information regarding your child’s illness when calling, stating if they have a fever and if there are any other symptoms. When able, please do NOT wait to report their illness until they return to school. Thank you for doing your part to help keep Potomac View students, families, and staff members healthy!
